I have been giving these out for years to other Riders I meet:
Medical Information Data Systems, Inc.
They're substantially cheaper if you buy in "bulk" - and what's a few bucks among potential friends?

Always carry a Pencil with you to fill these out - ink runs when it gets wet.

can you get the info ink'd on yer head if you don't wear a helmet?

I did see a living will ink'd on a guys forearm, it really looked good too, signed & everything, I guess it's legal as long as still legible from the roadrash.
